Please note these drugs are not a substitute for vaccination which remains the most effective way of preventing illness from influenza.
Oseltamivir and zanamivir reduce the replication of influenza A and B viruses. They are most effective for the treatment of influenza if started within a few hours of the onset of symptoms and usually within 48 hours. They can reduce the risk of complications from influenza in the elderly and those with chronic disease. They are also licensed for post-exposure prophylaxis when influenza is circulating in the community.
For otherwise healthy adults, antivirals are not recommended. Treat "at risk" patients when influenza is circulating in the community and within 48 hours of onset, or in a care home where influenza is likely.
Baloxavir is restricted to hospital use under Infectious Diseases / Microbiology guidance for the treatment of severe influenza in patients where neuraminidase inhibitors are contraindicated or resistance is suspected/confirmed, in line with UKHSA guidance
Post-exposure prophylaxis for persons in at-risk groups is not routinely commissioned and should only be used following specialist advice where resistance to neuraminidase inhibitors is confirmed in the index case.
