Linagliptin may be a good choice in patients with reduced renal function as no dosage reduction is necessary even in severe renal impairment. Other DDP-4 inhibitors may be used and all require dosage reduction according to creatinine clearance. Renal function should be measured before initiating treatment with a DPP-4 inhibitor and at regular intervals thereafter.
