Heparin-induced thrombocytopenia (HIT) is a serious complication of UFH treatment. It is characterised by mild to moderate reductions in platelet counts of delayed onset typically 5 to 10 days after heparin initiation or earlier in a patient who has previously been sensitised to heparin. It is associated with the paradoxical but frequent occurence of serious thromboembolic events. Serial platelet counts should be measured if heparin is given for longer than 5 days, (or sooner if previously exposed).

If HIT is suspected in a patient, heparin MUST be stopped and other options considered; this may include use of alternative anticoagulants.
