Kent & Medway Formulary - Key Application Forms

| 3 | Formulary Amendment Request Form | For new formulations, new indications, or status changes to existing drug entries |
| 4 | SBAR form | General explanation document to support other changes/guidance |
| 5 | Single Patient Request Form | One-off non-formulary request for a single patient only |

MTW - other local application Forms
Request form for a new Ready-to-Use product (Hospital Only). Use this form for requesting a new ready to use product at MTW for a drug that is already listed on the formulary.

Please note that formulary applications intended for use in both primary and secondary care will go through the K&M ICB governance committee process, not DTMMC. Please check with formulary team for clarification on process and dates.
